Snow- Per inch on the commercial accounts, per pass on the residentials
Salt- bulk sand/salt mix & set price per application, although I if use straight rock salt I charge accordingly.

Oh, I should add another thing that's important. We bill per storm, and the day after each storm (net 30)

All three,
Hourly, incremental, and seasonal. Hourly is charged right to Coprorate Amex day of storm.

We have all three. Even though residential customers makeup 90% of my client base, I don't plow any of them. Everything gets subbed out to a friend of mine who does roofing. I actually plow for my best friend and run operations for his company which consists of 10 trucks from 3/4 ton pickups to large 5 ton dumps.
For the BP's we have a seasonal contract with set plowing and salting prices. Everytime we plow, we salt. Under 2" the secretary makes calls and once we get 2 lots, I'm in the truck. I go from one end of town to the other and the secretary calls ahead of me to ask if they want salt. If the storm dumps over 8" we double our plowing rate. Once the storm is done, all the guys turn in their tickets and we bill through Amex. It can be a pain in the ass but we can bill 12 hours after we get done plowing and the money is in our account a couple hours after.
For the restaurants, Arby's and Mr. Chicken is under contract in which they get billed monthly and Wendy's is per push, billed monthly. We plow whenever we get 2" or more. For the Wendy's if it's questionable we plow it. None of the restaurants get salted unless requested by the managers. At that time we bill salting per yard. We charge $135 per yard put down.
Public Storages are under contract. Same contract as BP but we bill monthly instead of right after the storm. If we need to bring in a wheel loader to move piles we charge hourly.
All of our other accounts which include a couple factories and office buildings are per push and get billed monthly. Sidewalks get done after we're done plowing and get billed per event monthly.
